before doing anything else. Once you've read the notes, either complete the referral on this screen and click "submit",

Please note, some of our policies were amended on 4 February 2008. Please check to ensure you
are up to date with our operations, and to avoid and confusion or disappointment.
If your client requires a starter pack, please give details in the items required section. Please list the
number of people in the family (Male, Female and Children with ages), number of beds (single, double etc) and if curtains
are required, the quantity and size if known. An official form and addendum to this online referral will be uploaded
as soon as possible.
